2008 Tournament Entry Procedure

NOTE: If your team has participated in past years, you will find that the registration forms have changed. Please read these instructions before filling them out. Incomplete forms or forms without payment will not be accepted.

Entry forms must be received electronically, via fax or mail by close of business Friday, June 13.

Space is limited and entries will be accepted on a “first-come, first-serve” basis.

This is a non-sanctioned event. Player cards are not required. However, teams will be placed in divisions on their reported basis of competitive level, subject to verification.

Please download and read the 2008 Tournament Entry Procedure before filling out the forms.

Teams and players are registered on separate forms. This year we introduce automated Adobe PDF online forms that may be emailed directly to the Tournament Office. The forms require the Adobe Reader software which is available free from Adobe at: www.adobe.com/products/acrobat/readstep2.html

1. Team Registration Form - DOWNLOAD
Use this form to register your team (not the players). The registration fee must accompany this form. See Entry Fee below. Please register your team one time only. If changes to team information become necessary, please call or email your changes. Do not submit a new Team Registration form.

2. Team Roster Form - DOWNLOAD
Use this form to register your team's players. Please include signatures. Parent or legal guardian signature giving permission is required for every player under the age of 18. Teams and Players will not be registered without these signatures. Changes in players may be submitted using the Team Roster Form.

3. Medical Release - DOWNLOAD
A signed Medical Release is required of all players participating in the tournament.

What does it cost?
ENTRY FEE: A non-refundable entry fee of $125* must accompany all entries. You may pay by check, money order, or credit card (MasterCard, VISA, American Express or Discover). Do Not Send Cash. *(After May 23, entry fee increases to $150)

If paying by credit card, you may submit your information automatically online, by fax, or by mail. If paying by check or money order, please submit your forms by mail.

Refund Policy
Note: Entry fees for McDonald’s® McSoccerfest once received, are completely non-refundable, even in the case of event cancellation or rescheduling.

How to submit your entry forms:

Method 1. Complete the automated PDF forms onlline. Click the "Submit Form" button. (Your computer's email program will prompt you to send the form. All data will be sent directly to the McDonald's® McSoccerfest Tournament Office.)

Method 2. Print and complete them. Fax them to: 703-536-2255.

Communication
Communication between the tournament office and team captains will occur largely by email and all information will be posted on the official tournament Web site – www.mcsoccerfest2008.com – in imminent fashion. Each team must have an active email address to which information can be directed.
